We are seeking a highly skilled and compassionate Social Prescribing Link Worker to join our team at Andover Health Centre Medical Practice. Key Responsibilities- Community Outreach and Engagement
- Develop and maintain strong relationships with local community groups, voluntary sector organisations, and statutory agencies to promote social prescribing and encourage referrals.
- Build trust with patients and their families, providing person-centred support and empowering them to take control of their health and wellbeing.
- Referral and Signposting
- Provide advice and signposting services to patients, carers, and professionals, ensuring they receive the right support and resources.
- Work collaboratively with local agencies to raise awareness of social prescribing and promote partnership working to reduce pressure on statutory services.
- Community Development and Support
- Support local community groups and VCSE organisations to develop and sustain their services, ensuring they are accessible and effective.
- Encourage people to volunteer and give their time freely to others, building community resilience and promoting social connections.
- Data Capture and Evaluation
- Work sensitively with patients and their families to capture key information, enabling tracking of the impact of social prescribing on their health and wellbeing.
- Provide regular feedback and updates to referral agencies, ensuring they are aware of the effectiveness of social prescribing.
- Essential Qualifications and Experience
- GCSE level or equivalent in Mathematics and English.
- Relevant qualifications set out by the Personalised Care Institute or willingness to study for these.
- Experience of working directly in a community development context, adult health, and social care, learning support, or public health/health improvement.
- Desirable Qualifications and Experience
- Training in motivational coaching and interviewing or equivalent experience.
- Knowledge of the personalised care approach, community development approaches, and VCSE and community services in the locality.
- Personal Qualities and Attributes
- Ability to listen, empathise with people, and provide person-centred support in a non-judgemental way.
- Able to get along with people from all backgrounds and communities, respecting lifestyles and diversities.
- Commitment to reducing health inequalities and proactively working to reach people from all communities.
