We are seeking an experienced Practice Nurse to join our team at Primary Care Careers. As a Practice Nurse, you will play a vital role in providing specialist care to patients with chronic diseases, working autonomously and collaboratively with our healthcare professionals.
Main Responsibilities:
- Support the nursing team to provide a suitable learning environment.
- Work autonomously with patients with LTC chronic disease management following local and national guidance.
- Work collaboratively with a variety of healthcare professionals to deliver a high standard of nursing care to patients.
- Assess, develop, and implement nursing care programmes for patients with diabetes and provide specialist care to these groups of patients.
- Implement and evaluate individual treatment plans for patients with diabetes, that promote health and wellbeing.
- Work as part of the practice nurse team providing general practice nursing services to patients.
- Assist with the leadership and good management within the nursing team.
- Work autonomously following local and national guidance.
- Take cervical smears/triple swabs and interpreting and acting on swab results.
- Travel health advice and vaccination.
- Administer injections under individual patient prescription or patient group directives.
- Maintain safe storage, rotation and disposal of vaccines and drugs.

- Essential: RGN
- Desirable: Diabetes diploma, Accredited practice nurse qualification, Experience of working within primary care, Evidence of professional development, Respiratory and diabetes long term condition management experience.
