Salary:
The successful candidate can expect a competitive salary of £35,000 - £40,000 per annum, plus mileage allowance and full training opportunities.

Job Description:
As a Care Coordinator with our organization, you will be responsible for liaising with hospitals, GPs, social services, and families to ensure seamless care delivery. Your key duties will include conducting assessments and reviews, managing on-call duties, and maintaining strong relationships with stakeholders.

Required Skills and Qualifications:
To excel in this role, you will need a full UK driving license and access to a vehicle. A background in care coordination or a related field is also desirable.
