About the role:


We are seeking an experienced and skilled professional to join our Community Review Team (CRT) as an Assistant Practitioner. The OPPD Service supports older adults and individuals with physical disabilities, and we are a multi-professional service comprising Social Workers, Occupational Therapists, and Assistant Practitioners.


As an Assistant Practitioner, you will be responsible for conducting Care Act 2014 assessments to determine eligibility for services, including proportionate reviews, ongoing support, support planning, and working with complex issues. You will also complete Carers assessments to ensure the support of informal carers within the borough.


About you:


We are looking for resilient practitioners who can manage competing demands and prioritize work effectively. You will have experience working with older people and individuals with disabilities, as well as the ability to undertake strength-based social care assessments.


The role requires confident decision-makers who can remain calm under pressure. You will have experience working effectively with other professionals and partner agencies as necessary, and the ability to establish a rapport and build a professional, respectful, and honest relationship with service users and their families/carers.
