About the Role
We are seeking a highly motivated and experienced Support Coordinator to join our team in North Tyneside. As a Support Coordinator, you will play a vital role in supporting our customers with complex needs to develop their skills and live more independently in the community.
Key Responsibilities
- Creating personalized support plans with customers and working closely with social workers, community treatment teams, and other stakeholders to ensure comprehensive support.
- Enabling customers to participate in community activities, prompting with medication, supporting with finances, and assisting with daily living activities.
- Providing emotional support and guidance to customers, promoting their independence and well-being.
- Conducting risk assessments, support planning, and regular reviews to ensure customers receive the best possible support.
- Collaborating with colleagues to ensure seamless support and promoting a positive team culture.
Requirements
- Experience of creating person-centered support plans and supporting colleagues in working to these plans.
- Ability to work independently, remain calm under pressure, and have a resilient approach.
- Valid driving license and access to a vehicle insured for business purposes.
- Enhanced DBS check.
