Care Coordination Specialist

Holgate Primary Care Network (PCN) is a leading provider of healthcare services in Middlesbrough, serving a population of approximately 45,000 patients. We are currently seeking a highly organized and customer-focused Care Coordination Specialist to join our existing team working within our PCN Practices.

About the Role

The successful candidate will be responsible for providing direct support to the PCN Practices in delivering and coordinating care to our patients with a focus on our Quality Outcome Framework (QoF), PCN DES & GP LIS requirements. This includes coordinating the booking of identified patients into practice clinics, regularly monitoring appointment systems, and supporting admin teams in practices to telephone patients due for health checks or reviews.

Key Responsibilities
  • Coordinate the care needs of individuals in line with processes and requirements agreed by the PCN Management Team
  • Manage multiple tasks such as booking appointments, managing IT and remote booking systems, telephone skills, and record keeping
  • Coordinate the care of housebound and care home residents, preparing clinics, and carrying out follow-up calls with patients
Requirements

We are looking for an individual with exceptional organisation and customer care skills, empathy regarding patient needs, and the ability to manage multiple tasks simultaneously. A background in a health or medical environment is desirable but not essential. The successful candidate will be required to undertake training as set out by the Personalised Care Institute.

What We Offer

We offer a competitive salary of £25,500 - £30,000 per annum, depending on experience, plus benefits including opportunities for career development and a supportive work environment.
