We are working closely with a local authority in Enfield to appoint a Reablement Officer on a 2-month contract with potential extension. Apply with your CV for immediate consideration.
Job Description:Key Responsibilities:
- Work with service users to assess, design, plan, implement, and support care plans/management of cases as directed by the line manager.
- Establish, develop, and sustain effective working relationships with service users.
- Provide assistance and support to meet the needs and requirements of service users.
- Undertake key/case worker roles as agreed within the service.
- Assist in ensuring service users have access to advocacy and empowerment to self-advocate.
- Actively support and promote the inclusion and participation of service users.
Qualifications:
- A Social Care qualification (e.g., NVQ/QCF 3 in Social Care or equivalent).
Important Information:
- Availability to work immediately or at short notice.
- Right to work in the U.K.
- Enhanced DBS required.
