Job Description

We are seeking a caring and motivated Social Care Worker - Independent Living to join our team. In this role, you will have the opportunity to make a real difference in the lives of our clients, supporting them to maintain their independence and engage fully with their communities.

You will work closely with clients, their families, and carers to develop and implement personalized care plans that meet their unique needs and goals. Your excellent communication and interpersonal skills will enable you to build strong relationships with our clients and stakeholders, ensuring seamless service delivery.

Key Responsibilities
  • Deliver personalized care and support services to clients in their homes and within their communities
  • Develop and maintain strong relationships with clients, their families, and carers
  • Contribute to the development and implementation of client care plans
  • Participate in ongoing training and professional development to enhance skills and knowledge
Requirements
