Job Title: Adult Support Professional

We are seeking a highly motivated Adult Support Professional to join our Adults Locality Team in Kent. In this role, you will work closely with service users, their families, and other professionals to develop personalized care plans that promote independence and well-being.

About the Role

You will be responsible for assessing adults' needs, developing care plans, and managing a diverse caseload of adults with varying needs. You will also promote independence among adults by accessing community resources and promoting self-sufficiency.

Key Responsibilities
  • Conduct assessments of adults' needs and develop personalized care plans.
  • Manage a diverse caseload of adults with varying needs, including older adults, individuals with disabilities, and those with mental health concerns.
  • Promote independence among adults by accessing community resources and promoting self-sufficiency.
  • Collaborate with colleagues from various disciplines to provide holistic support to service users.
  • IDentify and respond to safeguarding concerns promptly, following local policies and procedures.
Requirements
  • Qualified social worker with 2 years plus experience in adult's social care.
  • Car driver.
  • Social Work England registered.

The estimated salary for this role is £38 per hour, recognizing the value of your skills and experience in delivering high-quality care and support to adults with varying needs.
