About the Role:

We are seeking a dedicated and compassionate Support Worker to join our team at Gray Healthcare Ltd. As a Support Worker, you will play a vital role in providing high-quality care and support to individuals with complex needs, including those with Autism and Moderate Learning Disability.

Key Responsibilities:

  • Providing 1:1 support to individuals in their own homes, promoting independence and well-being.
  • Assisting with daily living tasks, such as personal care and medication management.
  • Implementing detailed care plans and risk assessments to ensure the individual's safety and well-being.
  • Working collaboratively with a mixed-gender team to provide 24-hour support.

Requirements:

  • Previous experience in social care, ideally within mental health.
  • Ability to provide encouragement and support to individuals with complex health needs and Learning Disabilities.
  • Experience of implementing detailed care plans and risk assessments.
  • Current Right to Work in the UK and at least 6 months' employment in the UK.
