About the Role

We are seeking a skilled Mental Health Practitioner to join our Community Mental Health Team at the Lancashire and South Cumbria NHS Trust.

Key Responsibilities
  • Conduct comprehensive assessments and develop personalized care plans to support service users and their families/carers.
  • Provide evidence-based interventions and specialist advice to promote recovery and well-being.
  • Work collaboratively with a multidisciplinary team to enhance the unique value of mental health practitioners and promote a culture of excellence.
  • Deliver evidence-based interventions following NICE guidelines to promote recovery and improve outcomes.
  • Regularly review care and outcomes, supporting service users to complete clinical outcome measures and adapt treatment options as needed.
