Care Manager Lead

We are seeking an experienced and skilled Caseload Manager to join our Community Nursing Team in Deal, Kent. The successful candidate will be responsible for undertaking holistic assessments, planning appropriate care, and delivering clinical interventions to meet the various needs of patients on their caseload.
This is a fantastic opportunity for someone who is passionate about delivering high-quality patient care and has excellent clinical skills and knowledge of complex disease management. As a senior clinician, you will have a strong understanding of National Service Frameworks and NICE guidance relevant to the care setting. You will also have experience of managing health-related projects and complex caseloads, as well as supporting others through mentorship and education.
The ideal candidate will have a minimum of 5 years' experience in the area of practice and hold a full driving licence valid in the UK with access to a vehicle. They will also have a strong ability to motivate and organise others to ensure best practice, as well as manage and lead change effectively.
Key Responsibilities:
- Undertake holistic assessments and plan appropriate care for patients on the caseload.
- Deliver clinical interventions to meet the various needs of patients.
- Manage health-related projects and complex caseloads.
- SUPport others through mentorship and education.
- Registered healthcare professional (NMC registration)
- Full driving licence valid in the UK with access to a vehicle.
- Minimum 5 years' experience in area of practice.
