Job Summary

We are seeking a highly motivated and compassionate Care Coordinator to join our team at Pobl Group. As a key member of our Support Services team, you will be responsible for delivering high-quality support services to individuals in need.

In this challenging yet rewarding role, you will work closely with our clients to identify their needs and develop personalized support plans. You will also be required to maintain accurate records, collaborate with external agencies, and contribute to the development of new initiatives to improve service delivery.

Key Responsibilities:
  • Deliver high-quality support services to individuals in need
  • Develop and implement personalized support plans
  • Maintain accurate records and reports
  • Collaborate with external agencies and partners
  • Contribute to the development of new initiatives and programs

Requirements:
  • Degree in Social Work or related field (or equivalent experience)
  • Minimum 2 years' experience in social care or related field
  • Excellent communication and interpersonal skills
  • Able to work independently and as part of a team
  • Strong problem-solving and analytical skills
