At NHS, we are seeking a highly skilled Healthcare Navigator to join our team. The estimated salary for this role is £35,000 - £45,000 per annum.

The successful candidate will support the Site Capacity & Flow Lead in managing patient flow from acute providers to and across SCFT Intermediate Care Units. This will involve ensuring all bed capacity across the Trust is utilised whilst ensuring patients experience safe, high quality care throughout their patient journey.

This is an excellent opportunity for a dedicated individual to make a real difference in the lives of patients. As a Healthcare Navigator, you will work collaboratively with external stakeholders and partner organisations to address urgent care local pressure, system surges and escalation ensuring capacity is fully utilised and patient flow is maximised.

About the Role
  • Manage patient flow from acute providers to and across SCFT Intermediate Care Units.
  • Ensure all bed capacity across the Trust is utilised.
  • Work collaboratively with external stakeholders and partner organisations.
