Job Title: Support Worker Team Leader

We are seeking a highly motivated and experienced Support Worker Team Leader to join our team in Plymouth. As a key member of our team, you will be responsible for leading a team of support workers and working alongside them to provide person-centered and individualized packages of support to adults with a variety of disabilities.

About the Role:
  • Lead a team of support workers to provide comprehensive assessment and person-centered support planning
  • Assist individuals in identifying their needs and aspirations, and support them in formulating a plan to achieve their goals
  • Provide support with household tasks, social care tasks, educational/vocational development, and positive behavior development
  • Administer medications and maintain a safe environment
  • Manage rotas, record keeping, and team coordination

Requirements:
  • Previous experience in care and support work is essential
  • Full UK/EU driving license and own vehicle
  • Ability to work under pressure and in challenging situations
  • Self-motivated and passionate about the care of vulnerable people
