We are seeking a skilled and compassionate Community Mental Health Nurse to join our Memory and Assessment Service team in Coventry and Warwickshire Partnership Trust.
About the RoleThis is a Band 5 Mental Health Nurse position that requires a strong background in mental health settings, with a focus on working with older adults. The successful candidate will be responsible for delivering evidence-based interventions, conducting assessments and reviews, and developing care plans for individual service users.
As a member of our multidisciplinary team, you will participate in implementing, evaluating, and reviewing all aspects of service users' care. You will also provide supervision and support to junior staff, including Nursing Associates and support workers.
Key Responsibilities- Deliver day-to-day services to individual clients and their families and carers via a range of interventions.
- Agree course of action with those responsible for performing interventions, including supporting junior staff.
- Provide supervision and support to Nursing Associates and support workers.
- Conduct reviews, assessments, and plan care delivery for individual service users within the framework of the current Care Programme Approach.
- Enable service users to choose and participate in activities that are meaningful to them.
- Participate in the multidisciplinary team implementing, evaluating, and reviewing all aspects of service users' care.

- Knowledge of undertaking assessment, formulation, risk assessment, and positive risk management
- Proven experience of people management
- Evidence of continuing professional development
