Job Description

We are looking for an experienced Integrated Health and Wellbeing Assistant to join our team at Berkshire Healthcare NHS Foundation Trust.

This role is responsible for working closely with patients, families, and healthcare professionals to provide holistic support and guidance throughout the rehabilitation process.

Estimated Salary: £25,000 - £35,000 per annum

About the Job

The successful candidate will have a strong background in health and wellbeing, preferably with experience in rehabilitation or a related field. They will be able to demonstrate a deep understanding of the needs of patients with cardiac and respiratory conditions and possess excellent communication and interpersonal skills.

The primary responsibilities of this role include:

  • Providing individualized support to patients and their families
  • Collaborating with healthcare professionals to ensure comprehensive care
  • Developing and implementing exercise programs and activities tailored to each patient's needs
Requirements and Qualifications

To be considered for this position, applicants must have:

  • A relevant qualification in health and social care (e.g., NVQ Level 3 GP Referral Exercise Instructor)
  • Previous experience in a healthcare setting, preferably in rehabilitation
  • Excellent communication and interpersonal skills
