Chronic Condition Manager RN

Hinesassoc is seeking a skilled Registered Nurse to join our team as a Chronic Condition Manager. In this key role, you will utilize your clinical nursing experience to develop personalized plans for meeting the healthcare needs of patients, while continuing to grow in healthcare knowledge and perspective.
Key Responsibilities:
- Assess patient medical and psychosocial status, including environmental and cultural influences.
- Determine economically responsible options for meeting patient medical and care needs.
- Collaborate with physicians, providers, and patients to facilitate quality, timely, and efficient medical care.
- Implement disease management plans by coordinating resources, including community resources, healthcare providers, and significant others.
- Evaluate disease management plans and make modifications as necessary.
Requirements:
- Registered nurse with current unrestricted license to practice in the state or in a state with licensure reciprocation.
- Successful completion of disease management orientation program.
- Minimum of 2 years full-time equivalent of direct clinical care to the consumer.
- Basic typing/computer knowledge with minimum keyboarding speed of 35 WPM.
