About the Role:

We are seeking a highly motivated and compassionate Community Connexions Outreach Worker to join our team at Alzheimer Scotland. As a key member of our outreach service, you will play a vital role in supporting people living with dementia to achieve their personal goals and outcomes.

Key Responsibilities:

  • Provide guidance and assistance to people living with dementia and their carers to help them live life to the full and as independently as possible.
  • Help reduce social isolation, build confidence, and engage with the local community.
  • Work collaboratively with individuals and groups to develop and implement personalized support plans.
  • Provide emotional support and advocacy to people living with dementia and their carers.
  • Participate in relevant training and professional development opportunities to enhance your skills and knowledge.

Requirements:

  • Positive approach to dementia and a commitment to person-centered care.
  • Excellent communication and interpersonal skills.
  • Ability to work flexibly and independently in a fast-paced environment.
  • Relevant qualification and experience of working with people with dementia (desirable).
