Patient Empowerment Specialist

The Patient Empowerment Specialist will work in partnership with practice teams to support patients and promote lifestyle changes, preventing ill-health through behaviour change techniques. This role requires a good understanding of social prescribing, well-being, and community services, as well as motivational coaching and interviewing skills.
Responsibilities- Work 1:1 with patients to identify their needs and goals
- Provide person-centred support to empower patients to take control of their health and wellbeing
- Build trusting relationships with patients and practice teams
- Support patients to access community groups and statutory services for practical and emotional support
- NVQ Level 3 or equivalent qualifications
- Demonstrable commitment to professional and personal development
- Training in motivational coaching and interviewing or equivalent experience
The estimated salary for this role is £35,000 - £40,000 per annum, depending on experience. The post holder will also receive a comprehensive benefits package, including pension scheme, annual leave, and opportunities for continued professional development.
