Clinical Lead for Community Mental Health Transformation

We are seeking a highly skilled and experienced Clinical Lead to join our team at Southern Health NHS Foundation Trust. As a Clinical Lead, you will play a key role in shaping the future of our community mental health services, working closely with our urgent community response teams, community nursing and therapy teams, and colleagues in Hampshire County Council.
The successful candidate will have a strong background in community health or social care, with experience of working in leadership roles and across multiple organisations and IT systems. You will be responsible for leading the Enhanced Recovery at Home Service (ERS) and the phlebotomy team, providing a responsive, safe, effective, and caring service to patients requiring care/support/reablement.
You will work closely with our teams to support admission avoidance and complex discharges, optimising individuals' functional needs and independence. You will also be responsible for developing and implementing comprehensive training and competency programmes for our staff, enabling them to take physical observations and recognise risks to skin pressure areas.

Key Responsibilities- Lead the Enhanced Recovery at Home Service (ERS) and the phlebotomy team
- Develop and implement comprehensive training and competency programmes for staff
- Work closely with our teams to support admission avoidance and complex discharges
- Optimise individuals' functional needs and independence
- Develop and implement multi-disciplinary pathways to support patient choice, improve quality of life, promote self-management, and assure early intervention
- Registered General Nurse
- History taking and physical assessment course
- Independent and supplementary Nurse prescribing course
- Teaching and Assessing qualification ENB 998 or equivalent/mentorship qualification PETALS
