Job Description

We are looking for a qualified and experienced nurse to join our Hospital at Home service in County Durham and Darlington NHS Foundation Trust.

The successful candidate will have a strong background in elderly care and palliative care, with excellent communication and interpersonal skills. As a key member of our team, you will be responsible for delivering high-quality patient care in the comfort of their own homes, working closely with our Acute Frailty Teams to identify patients suitable for this service.

You will be required to travel within a geographical area, covering 7 days a week. In return, you will have the opportunity to develop your skills as an autonomous practitioner and work in a dynamic and supportive environment.

About the Service:

  • Delivering high-quality patient care in the comfort of their own homes.
  • Working closely with our Acute Frailty Teams.
  • Identifying patients suitable for this service.

Requirements:

  • NMC registration.
  • Relevant registered nurse experience.
  • Strong background in elderly care and palliative care.
  • Time management skills.

Salary:

$70,000 - $90,000 per annum (dependent on experience).
