Care Management Team Coordinator

We are seeking a highly skilled and motivated Care Management Team Coordinator to join our team at Cartrefi Cymru Co. This role offers a competitive salary of £38,000 - £45,000 per annum.
About the PositionThis is an exciting opportunity to work in a dynamic environment where you will be responsible for developing and promoting person-centered, outcomes-focused support to service users. As Care Management Team Coordinator, you will work closely with the Service Manager to ensure compliance with legislation and Cartrefi's policies.
Main Responsibilities:- Collaborate with the Service Manager to ensure a safe and effective service delivery.
- SUPPORT THE SERVICE MANAGER IN ENSURING COMPLIANCE WITH LEGISLATION AND CARTREFI'S POLICIES.
- Lead and manage the team, focusing on performance improvement and team development.
- Plan and implement person-centered support plans, aligning with individual goals and aspirations.
- Act as a key point of contact for service users, addressing concerns and providing emotional support when needed.
- Monitor and evaluate the effectiveness of support services, identifying areas for improvement.
- Relevant experience working with adults with learning disabilities in a social care setting.
- Demonstrated ability to work effectively within a team environment.
- Excellent communication and interpersonal skills.
- Good understanding of health and safety legislation and management principles.
- Flexibility to work varied shifts, including evenings and weekends, as required.
