Advanced Health Care Specialist

In Suffolk and North East Essex, we are seeking a skilled Advanced Nurse Practitioner to join our Integrated Urgent Care team. As part of the Unscheduled Care Coordination hub (UCCH), you will play a vital role in ensuring that patients receive the right care, in the right place, first time, every time.
Key Responsibilities- Work collaboratively with our multi-disciplinary team to identify cases held in the East of England Ambulance Service queue that may be better served by other services
- Complete remote consultations with patients and healthcare professionals, making onward referrals to system partners to manage patient care needs closer to home
- Contribute to quality assurance and patient safety by identifying and suggesting service improvements with your UCCH team members
- You will need good knowledge of local urgent care, primary care, and secondary care pathways and infrastructure
- Able to work collaboratively with SNEE system partners to help deliver better quality of care for patients
- Masters-level education or equivalent is essential, along with non-medical prescribing qualification and at least 6 months experience consulting and prescribing independently
This full-time position offers a competitive salary of £40,000 - £60,000 per annum, depending on experience, with opportunities for full or part-time work.
