Complex Care Manager Lead

Job Description
We are seeking an experienced Complex Care Manager Lead to join our team in Liverpool/Northwest. As a leader, you will inspire team members to reach their full potential and provide top-quality, individualized care.
About the Role
The successful candidate will be responsible for developing, reviewing, and implementing client care plans and risk assessments, ensuring compliance with CQC regulations and best practice guidance. You will also maintain high standards of documentation and record keeping in compliance with CQC, national legislation, and local standards.
Main Responsibilities
- Act as a role model, fostering a supportive environment to empower staff in delivering quality care
- Uphold internal and external standards, adhering to CQC regulations, contractual requirements, NICE guidelines, company best practices, and Equal Opportunities
- Take joint ownership of a caseload of complex clients and manage their care package holistically
- Develop, review, and implement client care plans and risk assessments following CQC regulations and best practice guidance
- Co-ordinate and lead the transition and ongoing continuity of all allocated complex care packages
Requirements
- Comply with the Company's procedures regarding Conduct and Confidentiality
- Take responsibility for personal health and safety, as well as the health and safety of others
- Continuously develop professional skills through relevant training and updates
