The Role
As a Lead Practitioner, you will be responsible for leading a team of Support Practitioners in delivering person-centered care and support to our service users. You will work closely with our Service Lead to ensure that our services are running within agreed budgets and in accordance with our contractual requirements.
Key Responsibilities
- Lead and inspire a team of Support Practitioners to deliver high-quality care and support
- Provide guidance and mentorship to Support Practitioners to ensure they are professional, accountable, engaged, and motivated
- Contribute to the development and implementation of service plans and policies
- Work collaboratively with other teams to ensure seamless service delivery
- Monitor and evaluate service performance, identifying areas for improvement and implementing changes as needed

You will be part of a dynamic and supportive team, working with individuals with varying levels of support needs. Our team is dedicated to delivering exceptional care and support, and we are committed to making a positive difference in the lives of our service users.
About You
We are looking for an experienced professional with a passion for delivering high-quality care and support. You will have a strong understanding of the Health and Social Care sector, including the SSSC Codes of Practice and Health and Social Care Standards. You will also have excellent communication and leadership skills, with the ability to work collaboratively with others.
