Community Care Team Lead

About the Role
Corserv is seeking a highly skilled and experienced Community Care Supervisor to lead our team of Support Workers and Senior Support Workers. As a Community Care Supervisor, you will be responsible for overseeing the day-to-day operation of our services, ensuring the highest quality of care is delivered to our service users.
Key Responsibilities
- Oversee the work schedules of Support Workers and Senior Support Workers, ensuring they are optimized at all times.
- Supervise and monitor allocated Community Support Workers, taking appropriate action as required.
- Liaise with Health and social care partners to process referrals in a timely manner.
- Participate in national and local development programmes, carrying out the Care Certificate training for new staff.
- Deliver ongoing local formal and practical training to all team members.
- Carry out initial visits to service users to produce detailed reablement/support plans.
- Undertake relevant training and assess for and provide low-level pieces of equipment to enable customers to remain safe and independent at home.
- Monitor the service provided to service users, ensuring regular feedback is given to the Registered Manager and Commissioners.
Requirements
- Minimum level qualification in Health and Social Care or equivalent.
- Experience in providing direct care in a work setting.
- Supervisory/Line Management qualification and/or demonstrable experience of leading a team in frontline care.
- Awareness of developments within Health and Social Care, multi-agency care issues, and relevant healthcare training.
