Main Responsibilities

  • Conduct structured medication reviews to optimize patient care
  • Review acute medicines requests to ensure timely and effective treatment
  • Process hospital discharge and clinic letters to ensure seamless patient care
  • Provide support and advice to patients with self-limiting conditions
  • Collaborate with the multidisciplinary Primary Care Team to improve health outcomes in long-term conditions
  • Perform other general tasks as required

About the Role

This is an exciting opportunity for an experienced Clinical Pharmacist to join our dedicated clinical team. As a part-time Clinical Pharmacist, you will have the opportunity to work closely with our healthcare professionals to provide high-quality patient care.

Requirements

  • Independent Prescriber Status
  • Previous GP Practice experience
  • Minimum of 3 years Post Graduate experience
  • Excellent clinical knowledge
  • Commitment to continuing professional development
  • Computer literacy
  • Strong clinical leadership, communication, and interpersonal skills
  • Proficiency in SystmOne
