About the Role

We are seeking a highly skilled and dedicated Community Nurse to join our team at Livewell Southwest CIC. As a Community Nurse, you will play a vital role in providing high-quality care and support to individuals with mental health issues and learning disabilities.

Key Responsibilities
  • Provide comprehensive nursing care to individuals with mental health issues and learning disabilities in a community setting.
  • Work closely with multidisciplinary teams to develop and implement individualized care plans.
  • Conduct assessments and evaluations to identify the needs of individuals and develop strategies to address them.
  • Provide education and support to individuals, families, and carers to promote independence and well-being.
  • Collaborate with other healthcare professionals to ensure seamless care and support.
  • Participate in quality improvement initiatives to enhance the quality of care and services.
Requirements
  • Registered Nurse (RGN/RMN) with a valid NMC registration.
  • Experience of working with individuals with mental health issues and learning disabilities.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work in a variety of settings, including community and inpatient environments.
