Job Title: Enhanced Care Home Team Nurse Educator

We are seeking a highly skilled and experienced Nurse Educator to join our Enhanced Care Home Team. As a key member of our team, you will play a vital role in providing leadership, education, and support to care homes, with a focus on improving the quality of care provision and reducing unnecessary hospital admissions.

Main Responsibilities:
  • Develop and deliver educational packages and advice to care homes on best practice and individual needs.
  • Assist in addressing specific patient needs, including review, advice, or signposting to other services where appropriate.
  • Work with all disciplines and professionals to educate, develop, and support clear understandings of patient needs and management within the care home environment.
Requirements:
  • Registered Nurse with current NMC registration.
  • Evidence of continuing professional education and training at degree level.
  • Post-registration qualification in a relevant field at specialist level or equivalent experience.
  • Recognised teaching qualification or equivalent experience as part of the role.

  • Ability to work autonomously and as part of a team.
  • Flexibility and adaptability to change and meet the needs of the service.
