**About the Role**

We are seeking a dedicated and compassionate individual to join our team as a Support Worker at Achieve Together. As a Support Worker, you will play a vital role in supporting people in their homes and local communities, promoting their independence and well-being.

**Key Responsibilities**

  • Provide person-centered support to individuals, promoting their physical and emotional well-being
  • Assist with daily living tasks, such as personal care and household chores
  • Encourage and support individuals to engage in their local communities and pursue their interests
  • Work collaboratively with colleagues and other professionals to ensure seamless support
  • Contribute to the development and implementation of individual support plans
