Pharmacist Job Opportunity in West Lothian

We are seeking a dynamic and enthusiastic pharmacist to join our team in West Lothian, delivering pharmaceutical care in both primary and secondary care.

This role is an exciting opportunity to work with our Hospital at Home and Community Hospitals team, providing holistic, patient-centred individualised care to improve the safety and effectiveness of individuals' medicines.

About the Role
  • Work closely with hospital at home and other teams in West Lothian as required.
  • Support the provision and development of pharmaceutical care initiatives in primary care, collaborating with primary care, social care, and community pharmacies.
  • Liaise with secondary care acute and St Johns Hospital teams.
  • Provide direct pharmaceutical care to older people with complex needs in primary care and community hospitals.

Requirements

To be successful in this role, you will need:

  • GPhC registration as a pharmacist.
  • Postgraduate experience working in a patient-focused clinical environment.
  • Excellent interpersonal and communication skills, with leadership and innovation consistent with NHS Scotland's values.
