The Specialist Practitioner will work within the ReConnect Care service, providing clinical expertise to individuals in prison and the wider community of Lancashire & Cumbria. The post holder will assess and support individuals with complex mental health needs, working closely with partner agencies to ensure effective care pathways.
Main ResponsibilitiesThe Specialist Practitioner will:
- Assess individuals with complex mental health needs, using validated assessment tools and applying knowledge of the criminal justice system.
- Develop and implement care plans, working collaboratively with other agencies to ensure effective support.
- Provide clinical oversight to the caseloads of other team members, promoting the independent self-management of service users' healthcare needs.
- Manage their own caseload, ensuring timely and effective support to individuals.

Person SpecificationThe ideal candidate will have:
- Knowledge of the Trust's information sharing protocols and compliance with data protection regulations.
- Experience of working within the criminal justice system or with specialist knowledge of working with groups disadvantaged by their involvement in the justice system.
- Post-qualifying experience within their field of specialism or 12 months relevant experience of providing care in a community, criminal justice or secure setting.
