Lancashire and South Cumbria NHS Foundation Trust is seeking a dedicated Mental Health Practitioner to join their team in providing high-quality care to children and families.
Job OverviewThis role involves working with young people who present mental health problems, particularly those at risk of or who have attempted deliberate self-harm. The successful candidate will provide holistic assessments and treatments, as well as liaise with other professionals and agencies to ensure comprehensive care packages for individuals and their families.
Key Responsibilities- Assume responsibility for managing a defined caseload, using specialist knowledge and advanced clinical reasoning to assess, plan, implement, and evaluate care programs.
- Take an active role in the assessment and treatment of young people with mental health issues, focusing on those at risk of or who have attempted self-harm.
- Collaborate with other professionals and agencies to provide effective communication and integrated care packages around the individual, their family, and carers.
- Maintain accurate clinical records, including electronic data entry and recording, report writing, and responsible exercise of professional self-governance in accordance with professional codes of practice/conduct.
- Exercise autonomous professional responsibility for the assessment, treatment, and discharge of children, young people, and their families/carers.
- Provide an assessment and therapy service based on the appropriate use, interpretation, and integration of complex data from various sources, including psychological assessments, self-report measures, rating scales, and direct/indirect structured observations.
- Conduct risk assessments and risk management for individual children, young people, and their families/carers.
- Act as an advocate to facilitate the views and choices expressed by children, young people, families, groups, and communities, where appropriate.
