Advanced Clinical Pharmacy Professional

As a Clinical Pharmacist Specialist at North Thames Genomics Laboratory Hub, you will be responsible for delivering safe and high-quality care to patients. You will work as part of a multi-disciplinary team to achieve this goal, supporting the Prescription Team with medication queries and prescription requests.
Key Responsibilities:- Prescribe and review medication that is appropriate to patient needs and in accordance with the latest evidence-based practice, national and practice protocols, and within the scope of qualified practice.
- Support the clinical team with the management of long-term conditions and chronic disease.
- Request blood tests, blood pressure, and other monitoring as part of safe medicines management and drug monitoring.
- Excellent verbal and written communication skills.
- Able to demonstrate good teamwork and collaboration.
- Commitment to ongoing professional development and education.
£65,000 - £85,000 per annum.
