About Us:

Shared Services Partnership is committed to supporting people's independence and wellbeing. Our Community Independence Service (CIS) is a multidisciplinary reablement team that provides short-term support for adults with complex health and social care needs.

The Role:

As an Advanced Practitioner Occupational Therapist, you will provide professional support, leadership, and direction to the multi-disciplinary team. You will play a vital part in ensuring older people and adults with physical disabilities have their voices heard and benefit from care that offers choice and control.

You will support the team manager in the day-to-day running of the Reablement service and take a lead in identifying and managing risk, including supporting residents with moving & handling, and safeguarding needs.

The role requires a qualified professional Occupational Therapist with current HCPC registration and a firm belief that every voice in a community matters. You will bring proven experience in frontline social care and as a practice educator, demonstrating real empathy and a collaborative spirit.

Key Responsibilities:

  • Provide professional support, leadership, and direction to the multi-disciplinary team
  • Ensure older people and adults with physical disabilities have their voices heard and benefit from care that offers choice and control
