Job Description

We are seeking an experienced Adult Case Manager to join our team in Islington.

The successful candidate will be responsible for coordinating care for residents, conducting quality conversations, and collaborating with social workers and other professionals to deliver exceptional care and support.

Key Requirements

  • A QCF Level 3 qualification in health and social care (or equivalent).
  • At least 2 years of experience in social care, health, or the voluntary sector.
  • Strong knowledge of legislation and guidance related to adult care.
  • Excellent communication, negotiation, and advocacy skills.
  • A proactive, solution-focused approach to challenges.

Salary Range: £25-30 per hour
