Job Title: Essex RECONNECT Worker

Join The Forward Trust, a social enterprise with charitable status, in empowering individuals to break cycles of crime and addiction. As an Essex RECONNECT Worker, you will play a vital role in supporting people released from prison to access health and care services, ensuring a smoother transition to community-based services.

Key Responsibilities:
  • Process referrals and provide updates to referrers on assessment outcomes.
  • Conduct in-depth needs assessments for all suitable referrals.
  • Maintain contact and support to service users as identified in assessments, preparing them for and post-release.
  • Support the coordinator in recruiting and training prison peer supporters.
  • Maintain excellent partnerships with external agencies and key stakeholders.
  • Implement a clear referral pathway, including Offender Management in Custody, Family/Self, Prison, and Healthcare.
  • Ensure a holistic service is delivered safely to clients, aligning with individual and team targets.
  • Collaborate with colleagues to discuss and share best practices, engaging with risk assessment processes to deliver high-quality services safely.
  • Make referrals to Forward Trust's Meet and Greet service.
  • Ensure service users are fully connected with local services in preparation for release into the community.
Requirements:
  • Experience in supporting vulnerable adults, potentially through volunteering, student work placements, or previous employment.
  • Aptitude for working with vulnerable clients.
  • Ability to receive feedback.
  • Commitment to personal learning and development.
  • Ability and willingness to learn.
  • Ability to work as part of a team.
  • An ability to set and maintain professional boundaries.
