We are seeking a dedicated individual to join our team as an Essex RECONNECT Trainee. This exciting opportunity will involve working with vulnerable clients, providing support and guidance to help them transition into the community.

Key Responsibilities

  • Process referrals and provide updates to referrers on assessment outcomes
  • Conduct in-depth needs assessments for suitable referrals
  • Maintain contact and support service users as identified in assessments, both before and after release
  • Deliver a holistic service safely, in line with individual and team targets
  • Make referrals to Forward Trust's Meet and Greet service
  • Ensure service users are connected with local services in preparation for release
