About the Role

We are looking for a highly skilled and experienced Adult Clinical Lead who will be responsible for monitoring the state of wellbeing of each resident, while being aware of any changes that may impact on their care and care plans. The successful candidate will act as a focal point for contact with residents, their relatives, and visitors, as well as health and social care professionals involved in their wellbeing.

Key Responsibilities
  • Prepare and implement effective care plans that meet the needs of our residents.
  • Monitor the wellbeing of each resident and identify any changes that may require adjustments to their care plan.
  • Act as a liaison between residents, their relatives, and healthcare professionals, ensuring effective communication and collaboration.
Requirements
  • NMC registration.
  • NVQ Level 3 or equivalent in care for care homes.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
What We Offer
