Job Summary

Island Health is seeking a highly skilled Clinical Pharmacist to join our team in a patient-facing role. The successful candidate will work as part of a multi-disciplinary team to manage chronic disease areas and provide clinical medication reviews to proactively manage patients with complex polypharmacy.

Key Responsibilities
  1. Patient-Facing Clinics: Conduct clinics for patients with single or multiple medical problems where medicine optimisation is required, such as COPD and asthma.
  2. Medication Reviews: Deliver pharmacy-related aspects of the Network DES/LES, including conducting structured medication reviews (SMRs).
  3. Medicine Optimisation: Review the ongoing need for each medicine, monitor needs, and support patients with their medicines taking, ensuring they get the best use of their medicines.
  4. Recommendations: Make appropriate recommendations to Senior Pharmacists or GPs for medicine improvement.
  5. Clinical Audits: Undertake clinical medication reviews with patients and produce recommendations for senior clinical pharmacists, nurses, and/or GPs on prescribing and monitoring.
  6. Caseload Management: Manage a caseload of patients with common/minor/self-limiting ailments while working within a scope of practice and limits of competence.
  7. Medicine Queries: Provide patient-facing clinics for those with medicine queries and answer relevant medicine-related enquiries.
  8. Medicine Safety: Identify medications associated with harm or unplanned hospital admissions through audits or reviewing discharge summaries.
  9. Medicine Reconciliation: Reconcile medicines following discharge from hospitals, intermediate care, and into care homes.
  10. Repeat Prescribing: Assist with the development of a repeat prescribing process at the practice.
  11. Patient Risk Assessment: Identify cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches.
  12. Pharmaceutical Advice: Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components.
  13. Medicine Data Analysis: Analyse, interpret and present medicines data to highlight issues and risks to support decision-making.
  14. Clinical Leadership: Provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
Requirements
  1. Registration: Mandatory registration with General Pharmaceutical Council (GPhC).
  2. Education: Masters degree in pharmacy (MPharm) (or equivalent).
  3. Training: Completed 18-month CPPE pathway (or equivalent) or actively working towards.
  4. Independent Prescribing: Independent prescriber (or actively working towards).
  5. Experience: Prior GP/clinical pharmacy experience.
  6. Knowledge: Competent with specialist in-depth knowledge for multiple chronic disease areas, with the ability to consult and independently prescribe.
Skills and Abilities

The successful candidate will be able to plan, manage, monitor, advise, and review general pharmaceutical care programmes for patients across core areas, including disease states/long-term conditions. They will be accountable for delivering professional expertise and direct service provision, using skills in a range of routine situations requiring analysis or comparison of a range of options. They will be able to recognise priorities when problem-solving and identify deviations from normal patterns, referring to seniors or GPs when appropriate. They will be able to follow legal, ethical, professional, and organisational policies/procedures and codes of conduct. They will involve patients in decisions about prescribed medicines and support adherence as per NICE guidelines.
