We have an exciting opportunity for a qualified Professional to join our Perinatal Team as a Community Practitioner in Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust.
Key Responsibilities- Develop and deliver individualised programmes of Care based on comprehensive Core Assessments.
- Work collaboratively with Service Users and their families to promote Recovery and Wellbeing.
- Manage Risks and escalate as necessary.
To be successful in this role, you will need to have:
- A strong understanding of Perinatal Care principles.
- Excellent communication and interpersonal skills.
The estimated salary for this position is between £31,000 and £41,000 per year, dependent on your level of experience.
