About the Role

We are seeking a highly skilled and experienced Community Mental Health Practitioner to join our Dementia Intensive Support Service team at Essex Partnership University NHS Foundation Trust.

Key Responsibilities
  • Provide rapid assessments and interventions for individuals living with dementia or suspected dementia in crisis situations.
  • Work collaboratively with multidisciplinary teams to deliver high-quality, evidence-based care and support.
  • Develop and implement care plans that meet the unique needs of each service user.
  • Supervise and manage junior team members, providing guidance and support as needed.
  • Contribute to the development and implementation of service improvements and initiatives.
Requirements
  • Registered Mental Health Nurse or Registered General Nurse with a strong background in dementia care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Knowledge of health legislation, principles of CPA and Care Management, Mental Capacity Act, and safeguarding.
  • Ability to prioritize tasks and manage multiple responsibilities in a fast-paced environment.
  • Current UK driving license and access to a car for work purposes.
