As a Lead Health Professional, you will work closely with our clients who have underlying health needs to access and maintain the healthcare they require. This will involve completing initial health screening tools, creating health support profiles, and making referrals to relevant services.
The ideal candidate will have a strong understanding of common health issues and the barriers to health access faced by people experiencing homelessness. They will also have excellent communication and interpersonal skills, with the ability to build trust with clients and advocate on their behalf.
Key Responsibilities
- Develop and implement health support plans for clients
- Work closely with clients to access and maintain healthcare services
- Collaborate with external services to overcome barriers to healthcare access
- Monitor and evaluate the effectiveness of health support plans
