Job Title: Field Care Manager

About the Role:

We are seeking a highly motivated and experienced Field Care Manager to join our team at VP Complex Care. As a Field Care Manager, you will be responsible for managing a combination of different care packages and clients living in their own homes.

Key Responsibilities:

  • Rota management and scheduling
  • Risk assessment and care planning to meet individual client needs
  • Supervision and appraisal of staff to ensure high-quality care delivery
  • Client visits, spot checks, and care reviews to ensure client satisfaction
  • Development of new care packages from local authority and private case management companies
  • Supporting compliance of our field staff to ensure regulatory requirements are met
  • Showing motivation and commitment to the role and encouraging this within the teams you are responsible for managing
  • Supporting complaint handling and problem-solving to maintain the quality of care delivered to our clients
  • Monitoring support packages, including reviewing support worker documentation
  • Guiding staff through their own professional development
  • Effective communication with clients, their families, staff teams, and our colleagues in the multi-disciplinary team
  • Participating in the on-call rota

Requirements:

  • 1 year of experience in providing care management
  • NVQ level 3 or higher in Health and Social Care
  • DBS and reference checks will be conducted for all applicants

What We Offer:

VP Complex Care offers a competitive salary range of £24,000 - £26,000 per annum, depending on experience. We also offer opportunities for professional development and a supportive team environment.
