Care Coordinator Job Description:

At Routes Healthcare (North), we're seeking a highly skilled Care Coordinator to join our team of dedicated healthcare professionals. As a Care Coordinator, you will play a pivotal role in scheduling and coordinating care visits to ensure our clients receive the best quality care when they need it.

We're looking for someone who is passionate about putting people first, with excellent organizational and time management skills, and a deep understanding of CQC regulations. If you're a team player with superb communication skills and the ability to coordinate a team of healthcare workers, we'd love to hear from you.

Key Responsibilities:
• Coordinating the delivery of care using our care scheduling system
• Adapting to changes in schedules, including staff availability and new clients
• Ensuring that healthcare workers attend staff meetings, supervisions, and training within CQC-required timescales
• Working together with your Service Manager and Complex Case Managers to deliver a high-quality care service
• Maintaining regular client contact, and ensuring person-centred care is delivered to clients
• Recording all concerns, complaints, and compliments
• Managing on-call duties on a rotating basis
