Are you a highly skilled Advanced Clinical Practitioner looking for a challenging role in our Out of Hours Services? We have an exciting opportunity for an experienced ACP to join our team at Primary Care 24 (PC24).

In this dynamic role, you will be responsible for assessing, diagnosing and providing advice, care and management to patients presenting with undiagnosed or undifferentiated illness via telephone consultations, or face-to-face assessments for patients requiring home visits.

As an integral member of our multidisciplinary team, you will demonstrate clinical reasoning, critical thinking and autonomous practice, supported by GP colleagues in the out of hours setting.

  • Undertake face-to-face and telephone triage consultations
  • Interpret pathology/radiology results/reports
  • Prescribe and review medication
  • Plan, implement and evaluate an effective management plan for the patient

About the Role

We are seeking an experienced Advanced Clinical Practitioner who can deliver high-quality individualized care to our patients with a high level of autonomy. The successful candidate will have excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
