Job Summary:
Thames Reach is seeking a Support Worker to join our Hounslow Reach Floating team. As a Support Worker, you will work with individuals to sustain their housing and provide support to help them achieve their goals.
Key Responsibilities:
- Visit clients in their own homes and across the community, offering 1-1 tenancy-related support
- Deliver support to a caseload of clients with complex and multiple support needs
- Use a trauma-informed and co-produced support approach
- Work collaboratively with the people we support to create and review Risk Assessments and Support Plans
- Work in partnership with colleagues and other services to deliver holistic, co-ordinated support to clients
- Contribute towards service evaluation and improvement through engagement with the people we support, with colleagues, and with other key stakeholders
Requirements:
- Experience in mitigating and reducing risk to clients caused by health, safeguarding or welfare concerns
- Ability to be open to learning and using Trauma-Informed and Strengths-Based approaches to motivate and enable the people we support to achieve their goals and develop their resilience
- Experience in being reflective, flexible and responsive in meeting the needs of the people we support
- Ability to be confident in advocating for the people we support through joint work with partner services
- Ability to promote and positively represent Thames Reach in meetings and events with external services
