We are seeking a highly skilled Community Nurse to join our team at Unity Healthcare Recruitment. Based in Shropshire, this role involves delivering exceptional nursing care to patients in their own homes or community settings.

The ideal candidate will possess strong clinical skills, excellent communication, and a passion for patient-centred care. You will work collaboratively within a multidisciplinary team to provide high-quality nursing interventions and support the holistic needs of patients with acute, chronic, or complex health conditions.

  1. Clinical Care
  • Deliver a range of nursing interventions, including wound care, medication administration, catheter care, and end-of-life care.
  • Conduct holistic assessments of patients' physical, emotional, and social needs to develop and implement individualised care plans.
  • Monitor and evaluate patients progress, adapting care plans as necessary to achieve the best outcomes.
  • Provide health education and advice to patients and carers to support self-management and promote independence.

In return for your expertise, you can expect a competitive salary of £28,000 - £34,000 per annum, depending on experience.
