Job Summary

We are seeking a skilled and compassionate Support Practitioner to join our team. As a Support Practitioner, you will work closely with individuals to understand their unique needs and develop personalized support plans that promote their independence and well-being.

Key Responsibilities

  • Conduct thorough assessments to identify service users' needs and goals
  • Develop and implement tailored support plans that promote independence and well-being
  • Provide emotional support and guidance to service users, empowering them to make informed decisions
  • Collaborate with internal teams and external partners to ensure seamless service delivery
  • Monitor and evaluate the effectiveness of support plans, making adjustments as needed
